Prequalifications Requirements
- Completion of Trade Partner Qualification Form provided by estimating department with bid information
- Two most recent financial statements with a balance sheet, income statement, and statement of cash flows
- Current work in progress
- Banking limit and current balance
- Letter of bondability providing single and aggregate limits
- Project references for current work in progress with contract amounts
- EMR letter from workers compensation insurance carrier listing the most recent 3 years EMR rates
- Safety/Hazardous program
- Quality Management program
- OSHA Logs for the previous 3 years
- Current Insurance Certificate
